Patient Rights

As a patient, you have the right to:

  1. Respect & Dignity – Be treated with courtesy, respect, and without discrimination.
  2. Privacy & Confidentiality – Have your health information protected as required by HIPAA and our Privacy Policy.
  3. Informed Care – Receive clear information about your diagnosis, treatment options, and the risks and benefits of care.
  4. Consent – Accept or refuse telehealth services, treatment, or medication, and to be informed of alternatives.
  5. Access to Information – Request and receive copies of your medical records.
  6. Quality Care – Expect that licensed providers will deliver care that meets professional standards.
  7. Communication – Ask questions and receive understandable answers about your care.
  8. Complaints – Voice concerns or complaints without fear of discrimination or retaliation.
  9. Continuity of Care – Be referred for in-person care or emergency services when needed.
  10. Cost Transparency – Be informed of the fixed price for services before receiving care.

Patient Responsibilities

As a patient, you are responsible for:

  1. Accurate Information – Provide complete and truthful details about your health, symptoms, and history.
  2. Technology Access – Ensure you have a private, secure internet connection and functioning device for telehealth visits.
  3. Participation in Care – Follow treatment plans, prescriptions, and recommendations provided by your clinician.
  4. Respect for Providers – Treat providers and staff with courtesy and respect.
  5. Safe Use of Telehealth – Avoid using telehealth while driving or in unsafe environments.
  6. Payment – Pay for services at the agreed fixed price at the time of care.
  7. Emergency Awareness – Understand that telehealth is not for emergencies and call 911 or go to the nearest emergency department if needed.
  8. Compliance with Laws – Use services only for lawful purposes and in compliance with applicable regulations.
